Position Purpose: Perform triage and intake for incoming requests into case management and support services.Position Responsibilities:* Complete health risk assessments via telephone* Develop transition plans of care and review utilization data for assignment to service coordination* Conduct thorough and sound clinical and functional review of member’s needs* Assess level and assign to appropriate service coordination team within established timeframes* Maintain confidentiality of all caller and personnel issues* Document all intakes according to department policies and procedure Knowledge/Experience: LPN with 3+ years of clinical nursing experience or RN with 2+ years of clinical nursing experience.License/Certificates: Current state’s RN or LPN license
